Essential Lifestyle Planning Training
We have recently been training up about 32 Essential Lifestyle Planning Facilitators in Chorley and South Ribble, with the help of Gill Bailey from Helen Sanderson Associates.

Essential Lifestyle Planning is an approach to Person Centred Planning that is based on working in partnership with the person and those who know them best and care about them most.
It aims to listen to what a person is telling us with their communication about what is important to them, gather information together and take action that enables the person to achieve a lifestyle where they stay healthy and safe on their own terms.
The person and their circle of support work together to find ways to deliver changes that lead to improved quality of life, and deeper connections with the community.

It is an approach that values constant listening and learning, using Person Centred Thinking tools to reflect on what works and what does not work to deliver what is important to the person and ensure they get the kind of support that works for them.

The key aspect of Essential Lifestyle Planning is that it focuses on the perspectives of the person and those who care about them most.

Using the Person Centred Thinking Tools from Essential Lifestyle Planning helps services and supporters to:
* Focus on what is important to and for the person. * Define staff roles and responsibilities in a way that reflects the person's own priorities. * Match people with supporters who have the right personality characteristics, skills and shared interests. * Listen to a person's particular methods of communication. * Continue to learn, adapt, change and grow. | .jpg)
